GP and Specialist Referral 

A current GP referral is recommended to claim your Medicare rebate for your consultation. Please ensure your referral is sent to us prior to your appointment or brought with you on the day. Referrals from General Practitioners typically last 12 months, while specialist-to-specialist referrals are valid for 3 months.

Patient self referral is also accepted, however will not attract any Medicare rebate.

What to Bring

A Note on Your Treatment History

To help Dr Nick understand your journey, please list any previous treatments you have tried or bring any Specialist letters you may have with you to your appointment. This includes topical creams, ointments, and oral medications. Knowing what has or hasn’t worked in the past allows us to develop the most effective care plan for you.

Current Medications

Please provide a comprehensive list of all current medications, including dosage and frequency. This includes prescribed drugs, over-the-counter vitamins, and supplements to ensure your safety and treatment accuracy.

Health Records

Remember to bring your valid Medicare card and any relevant pathology or imaging results from the last 12 months. Having your previous health records and clinical history available facilitates a more effective consultation.

Plan Your Visit

Tips Before Your Appointment

To ensure your consultation runs smoothly, please leave enough time to complete medical forms and arrive a little early. If you are running late or need to reschedule, please notify the clinic as soon as possible. Remember to have your Medicare details ready so we can assist with your rebate.
